1. Clients may contact us directly. We ask that all clients fill out the “New Patient” form found on our website.

2. In the form, clients are asked to request any relevant medical records, diagnostics including images, and treatment history that can be provided via email at office@restorevetrehab.com.

3. We schedule the patient, perform the evaluation or therapy, and share progress updates at your request.
